HOW TO APPLY?

Fill up the application form and send it along with the following –

Covering letter 

Each entry must have a covering letter of approximately 500-1000 words describing in brief your practice/case/category and reasons why you should be selected for the category. Make your points loud and clear in your letter. Support it with evidences like a few testimonials and patient care factors. It would be best if you can present this information in power point format.

Photos and presentation 
Include as many good quality photographs as you can to impress the judges with your work. Try to stand out in the crowd with beautifully designed videos supporting your category. Before and after pictures of the cases will be required.
